Notice of Privacy Practices - HIPAA

·       BSW has a long-standing commitment to protecting the privacy of their patients’ health information. Part of this commitment involves compliance with the privacy standards contained in the regulations under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), the first comprehensive federal protection of health information.

·       HIPAA requires appropriate protections for the privacy of personal health information, and sets limits and conditions on the uses and disclosures that may be made of such information without patients’ authorization.

·       Our uses and disclosures - We may use or share your medical records or other health information as we: Treat you, bill your services, research, help with public and safety health issues, comply with the law and respond to lawsuits or legal actions.

·       Your rights - You have the right to: get a copy of your medical records, request confidential communication, ask us to limit the information we share, get a list of whom we have shared your information, and file a complaint if you feel like your privacy rights have been violated.

·       Your choices - In these cases, we will never share your information unless you give us written permission:  family, friends or others involved in your care, sale of your information or marketing purposes.
